 I was able to narrow it down:

 The output file is not broken, when I copy video and audio stream.
 However, it is broken, when I convert the audio. The target does not
 matter. I tried ac3 and mp3.

 This brings me to the conclusion that the mkv container is fine, but that
 the aac decoder has a problem.
